In December 2020, a worker was seriously injured after falling almost five metres from a stillage container that was being lifted by a forklift. The worker was repairing a jammed roller door when the container tipped, causing a dangerous fall.

To prevent similar accidents, workplaces must follow safe systems of work, including:

  • Only use approved forklift work cages that meet Australian Standards (AS 2359).
  • Secure cages properly with at least two locking devices.
  • Ensure the forklift operator remains at the controls.
  • Never use a work cage to transport people.
  • Always test-lift the cage without people first to confirm stability and clearance.
  • Workers must stand flat on the cage floor—not on ladders or objects.
